"Living with dying": The evolution of the family members' experience of mechanical ventilation.

BACKGROUND:: Communication with families about mechanical ventilation may be more effective once we gain a better understanding of what families experience and understand about this life support technology when their loved ones are admitted to the intensive care unit (ICU).

METHODS:: We conducted in-depth interviews with family members of 27 critically ill patients who required mechanical ventilation for >/=7 days and had an estimated ICU mortality of >/=50%. Team members reviewed transcripts independently and used grounded theory analysis.

RESULTS:: The central theme of family members' experience with mechanical ventilation was "living with dying." Initial reactions to the ventilator were of shock and surprise. Family members perceived no option except mechanical ventilation. Although the ventilator kept the patient alive, it also symbolized proximity to death. In time, families became accustomed to images of the ICU as ventilation became more familiar and routine. Their shock and horror were replaced by hope that the ventilator would allow the body to rest, heal, and recover. However, ongoing exposure to their loved one's critical illness and the new role as family spokesperson were traumatizing.

CONCLUSIONS:: Family members' experiences and their understanding of mechanical ventilation change over time, influenced by their habituation to the ICU environment and its routines. They face uncertainty about death, but maintain hope. Understanding these experiences may engender more respectful, meaningful communication about life support with families.

Objective: Intensive care unit patients are at particular risk for pressure ulcers and ventilator-associated pneumonia. Current guidelines recommend that mechanically ventilated patients be kept in a semirecumbent position with the head of bed elevated 30°–45° to prevent aspiration and ventilator-associated pneumonia. We tested the effects of elevating the head of bed on the interface pressure between the skin of the sacral area and the bed with healthy volunteers.

Coronary heart disease (CHD) is the leading cause of death in the United States, and unstable angina (UA) and non-ST-segment-elevation myocardial infarction (NSTEMI) are often acute manifestations of this disease. Each year, UA and NSTEMI affect more than 1.2 million people in this country, causing significant morbidity and mortality.

In 2007, the American College of Cardiology (ACC) and American Heart Association (AHA) released updated guidelines for the management of patients with UA and NSTEMI, revised from previous guidelines due to a growing body of evidence from clinical trials. The new guidelines were developed by experts from the ACC and the AHA, in collaboration with the American College of Emergency Physicians. The writing committee performed a literature review and made recommendations based on the strength of existing evidence. The committee created classifications (classes I, IIa, IIb, and III) and levels of evidence for each recommendation in order to estimate both the size and certainty of an effect for each recommendation.

PURPOSE OF REVIEW: The purpose of this review is to examine some of the latest concepts in the management of sepsis and septic shock in the Intensive Care environment, with a focus on Gram-negative infections.
RECENT FINDINGS: Recent developments in adjunctive therapies for sepsis and septic shock are discussed including protocol-driven therapies, the importance of timely appropriate antimicrobials and the growing threat and implications of rising Gram-negative antimicrobial resistance.

SUMMARY: The balance between the most effective and the most responsible prescribing is difficult to strike, but recognition of the impact of antibiotic failure makes it even more important to consider how we husband the microflora of the ICU. New therapeutic options are somewhat limited but goal-directed protocol-driven approaches linked with sound knowledge of the microflora and our impact upon it as an ecosystem is essential.PMID: 18978528 [PubMed - in process]
